CIGNA Individual and Family Plans

Our new Individual and Family Plans offer a simple-to-use, comprehensive health plan designed to provide you and your family with the comfort and peace of mind that comes with dependable health coverage.Eligibility Criteria

  • Individuals age 64 1/2 or younger on the assigned effective date
  • Children without an adult subscriber who are 3 months of age or older, up to age 19, regardless of student status
  • You must be a resident of the state of Arizona
  • HMO plan participants must reside in the Arizona Service Area for at least nine (9) months per Plan Year

Dependent Eligibility Criteria

  • Your lawful spouse of the opposite sex
  • Your unmarried children, stepchildren, adopted children, who have not yet reached age 19
  • Your unmarried children, stepchildren, adopted children who are at least 19, but under age 23 and are Full Time Students. Proof of student status may be required.
  • Your own or your spouse's unmarried children with continuing mental retardation or physical disability, regardless of age, enrolled prior to age 19. Proof of disability and dependency is required.

  • HMO Plan - comprehensive coverage, great value and predictable costs, along with the convenience of a PCP to coordinate your care.
  • Open Access Plan (New!)- flexible in and out-of-network coverage under a national network. No PCP or referral required.
  • Health Savings Plan (New!) - high deductible plans allows you to use a tax-advantaged Health Savings Account (HSA) to help pay for your current medical expenses or save for future medical expenses. In and out-of-network coverage under a national network. No PCP or referral required.

* Several states have not enacted legislation to allow pre-tax treatment of HSA contributions or earnings. You should work with your tax advisor to ensure that you are aware of any changes that may occur in these states. Health Reimbursement Arrangement

That's the idea behind the CIGNA Choice Fund Health Reimbursement Arrangement (HRA) – a plan that covers employees through an account funded by employer contributions.

Within the HRA, you also can choose from a broad array of innovative plan design options. For example, unused money can roll over into the next plan year, following limitations you select. And you can choose to include incentives for improving member health and well-being.

Flexible Spending Account

With an FSA, participants set aside pre-determined, pre-tax dollars, then use them to pay for eligible medical, dental and pharmacy expenses. Health Care Flexible Spending Accounts are a cost-effective way for participants to manage eligible out-of-pocket expenses not covered by medical plans. The participant pays no taxes on the contributions or the withdrawals.

CIGNA HealthCare offers two types of Flexible Spending Accounts:

Health Care Reimbursement Accounts
For health care expenses not covered by health benefits plan, such as copayments, coinsurance, deductibles or certain vision, hearing or orthodontic care costs. Participants do not need to be in a CIGNA HealthCare health benefits plan to be able to participate in a Health Care Reimbursement Account.

Dependent Day Care Reimbursement Accounts
For non-medical day care expenses for children ages 12 and under, or disabled dependents who satisfy the definition of a "Qualifying Relative" under federal tax law. Dependent day care expenses are reimbursable as long as the provider is not a spouse, another dependent, or your child if age 19 or younger.

Note: Our CIGNA Choice Fund HRA and HSA products are currently offered to employer groups with 200 or more employees and in certain states to employers with 50 or more employees. However, our CIGNA Choice Fund products are not currently offered to individuals or individual families.
